(2) Krishna River directly feeds Kolleru Lake: WRONG. Kolleru Lake in Andhra Pradesh lies BETWEEN the Krishna and Godavari deltas. It is fed by the Budameru and Tammileru rivers, NOT directly by the Krishna or Godavari. Krishna flows past Vijayawada south of Kolleru.
(3) Meandering of Gandak River formed Kanwar Lake: WRONG. Kanwar Lake (Kanwar Jheel) in Begusarai district of Bihar is an OXBOW lake formed by the BURHI GANDAK (a tributary of the Ganga), not the Gandak River. Strictly speaking the question may treat Burhi Gandak as separate from Gandak.
